Summary Care Records are uploaded to the National Spine from this practice.

The Summary Care Record is an electronic record which contains information about:

  • Any acute medication that you are taking
  • Any current repeat medication that you are taking
  • Any allergies that you have

You can view your own summary care record by accessing online services. If you are not registered for online services you will need to do this first. Find out more…

Only healthcare staff involved in your care can see your Summary Care Record. This should only be viewed in emergency or urgent care settings with your consent unless you are unable to give consent, for example, if you are unconscious.

If you want a Summary Care Record, you do not need to do anything.

If you do not wish to have a Summary Care Record, please complete the Opt-Out Form, or ask a member of our reception team for a form.

You are free to change your decision at any time by informing us.
